The successful candidate will play a crucial role in shaping patient-centred integrated care pathways, ensuring seamless transitions between hospital admission and discharge. This involves utilising high-level decision-making skills, clinical judgment, and providing expert advice to healthcare professionals, agencies, carers, clients, and relatives.

Key responsibilities include establishing, implementing, monitoring, and evaluating discharge management and education processes in collaboration with hospital multidisciplinary teams, discharge hubs, facilitators, and primary care providers.

This role requires membership in the PVG Scheme; if not already a member, an application will need to be made to Disclosure Scotland before starting the position.

We are also reducing our full-time working week from 37.5 to 37 hours per week, effective April 1, 2026. Part-time staff will receive a corresponding reduction in working hours without change in pay. However, some service areas may require additional temporary work up to 30 minutes per week until their rosters or working patterns adjust to the new schedule.
