Aged Services Emergency Team Care Coordinator - Allied Health
ASET is based in the Emergency Departments (ED) of AWH.
ASET Care Coordinators are responsible for providing expert, appropriate and coordinated screening, comprehensive assessment and care planning of people over 60 years of age, with the aim of preventing otherwise avoidable re-admissions to AWH and re-presentations to EDs, and contributing to the effective flow of patients through EDs.
ASET works with other AWH and non-AWH services to provide clinical/professional leadership in advocating for the care of older people, the development of appropriate clinical pathways, protocols, policy and models of care, which are informed by evidence-based practice and local clinical research.
Key Responsibilities:
Provide comprehensive assessment and care planning of people with complex needs.
Work collaboratively within interdisciplinary teams.
Communicate effectively with clients, carers and health professionals.
Develop plans that do not rely on acute hospital care to achieve optimal client outcomes.
Candidates meeting the following mandatory criteria are encouraged to apply:
Tertiary qualification(s) in Allied Health and eligibility for registration with AHPRA or other relevant peak professional body.
Clinical knowledge and experience in comprehensive assessment and care planning of older people.
Demonstrated ability to work collaboratively within interdisciplinary teams.
Demonstrated ability to communicate effectively with clients, carers and health professionals.
At least seven (7) years relevant clinical experience.
Current and valid drivers' licence.
