Position Overview:

Job Description:

The Care Buddy or Wound Care Nurse is responsible for the comprehensive assessment, treatment, and management of patients with acute and chronic wounds. This role requires developing individualized care plans, coordinating with a multidisciplinary team, and educating patients and their families on effective wound care and prevention strategies.

Key Responsibilities:

Wound Assessment and Management:
• Conduct thorough assessments of wounds, noting size, depth, stage, and any signs of infection.
• Develop and implement personalized treatment plans tailored to each patient’s specific needs.
• Perform advanced wound care techniques, including debridement, dressing changes, and application of specialized wound care products.

Patient Care and Education:
• Educate patients and their families on proper wound care techniques, infection prevention, and lifestyle modifications to enhance healing.
• Provide emotional support to patients and address any concerns related to wound management and recovery.
• Monitor patient progress closely and make necessary adjustments to treatment plans to optimize healing outcomes.

Collaboration and Coordination:
• Collaborate with physicians, nurse practitioners, and other healthcare professionals to ensure a holistic approach to wound care.
• Coordinate with other departments such as physical therapy and dietary services to support overall patient health.
• Participate in case conferences and multidisciplinary team meetings to review and discuss patient care and treatment options.

Documentation and Compliance:
• Maintain accurate and comprehensive patient records, including detailed wound assessments, treatment plans, and progress notes.
• Ensure adherence to healthcare regulations, infection control practices, and facility policies.
• Document any changes in patient condition promptly and report significant findings to the appropriate healthcare team members.

Quality Improvement:
• Engage in quality improvement initiatives and audits related to wound care practices.
• Stay informed about the latest advancements in wound care techniques and technologies to ensure the delivery of evidence-based care.
• Contribute to the development and revision of wound care protocols and policies to enhance patient outcomes.

Qualification:

Education: Registered Nurse (RN) with a current state license; specialized certification in wound care (e.g., Certified Wound Care Nurse (CWCN)) preferred.

Experience: Minimum 1 year of experience in wound care or a related field.

Skills:
• Expertise in advanced wound care techniques and knowledge of specialized wound care products.
• Strong communication and interpersonal skills for patient education and teamwork.
• Ability to assess patient needs and develop effective treatment plans.
• Proficiency in maintaining accurate documentation and adhering to compliance standards.

Additional Requirements: Ability to work in a fast-paced environment and manage multiple priorities. Flexibility to adapt to changing patient needs and treatment plans.
