Job Description

We are seeking a skilled Clinical Case Manager to join our team in Canberra. As a Clinical Case Manager, you will oversee the delivery of care to residents, developing and implementing personalized care plans in partnership with residents, their families, and healthcare professionals.

Your Key Responsibilities

Assess and develop care plans that meet the unique needs of each resident
Collaborate with healthcare professionals to ensure seamless care delivery
Maintain accurate records of care plans and resident progress
Requirements

Bachelor of Nursing degree
Current AHPRA registration
Demonstrated experience as a Registered Nurse within aged care
Knowledge of relevant legislation and regulatory requirements
