We are seeking a dedicated Clinical Assessment Coordinator to help individuals with complex needs transition to community living. In this role, you will conduct in-person interviews and assessments to evaluate the needs, strengths, and preferences of individuals, ensuring they can live safely and successfully in the community.

This position involves travel up to 85% of the time to facilities across Illinois for assessments.

Key Responsibilities
• Conduct medical and/or behavioral health assessments per contract guidelines.
• Perform on-site, video call, or telephonic assessments to determine service needs, eligibility, level of care, or related outcomes.
• Comply with Person First standards, HIPAA, and confidentiality policies.
• Travel to designated locations within Illinois to conduct in-person evaluations.
• Maintain detailed and accurate records, adhering to all professional standards.

Preferred Knowledge & Skills
• Familiarity with community support programs and long-term care assessments.
• Knowledge of medical and/or behavioral health conditions and medications.
• Strong data collection and analytical skills to assess individual needs.
• Excellent verbal and written communication.
• Strong interpersonal skills with top-notch customer service ability.
• Proficiency in Microsoft Office Suite.
• Organizational skills with great attention to detail.
• Ability to work independently in a fast-paced environment and meet deadlines.

Minimum Requirements
• High School Diploma or equivalent (required).
• Minimum 2 years of clinical experience.
• Licensure as an LPN, RN, LCSW, or equivalent based on contract requirements.
• Master’s Degree in Social Work, Mental Health, or a related field (preferred).
• 5+ years of post-graduate experience in medical or mental health fields.
• 3+ years of experience conducting medical or mental health assessments.
• Proficiency with Microsoft Office.
• Willingness to travel up to 85% of the time.

Home Office Requirements
• Company-issued computer equipment will be provided.
• Reliable high-speed internet with at least 20 Mbps download and 5 Mbps upload speeds.
• Private and secure workspace for conducting assessments.
