Clinical Case Manager Role

We are seeking a Registered Nurse to fill a permanent full-time position as a Clinical Case Manager.

About the Role:

Assess and develop personalised care plans for residents, ensuring their physical, emotional, and social needs are consistently met.
Maintain accurate records of care plans, government-funded home care packages, health status changes, and any interventions provided.
Collaborate with key stakeholders to address and support resident care needs.
Work with the lifestyle team to design and implement social, community service, and activity programs that align with residents' goals and enhance community connection.
Partner with doctors and allied health providers to ensure the continuity and quality of care for residents.
Requirements:

Bachelor of Nursing
Current AHPRA registration
Demonstrated experience as a Registered Nurse within aged care
Knowledge of relevant legislation, regulatory requirements, professional standards, policies and procedures
Strong written and verbal communication skills
National Police and NDIS clearances
Flu vaccination and Covid vaccinations
