Job description
An exciting opportunity has arisen to join the SPC team within Mersey Care NHS Foundation Trust as a Clinical Team Leader.
The Single Point of Contact is a busy and exciting service within the Trust. We are an established, innovative team of nurses and call handlers responsible for facilitating discharges into community settings. We signpost patients and health professionals to the appropriate setting utilising our knowledge base of pathways working with the many services across north Merseyside.
We are looking for enthusiastic and committed health care professionals with flexibility in working hours.
The post holder will have responsibility for the delivery of the clinical service in the defined area. The post holder will be required to provide team leadership and management to a team.
The primary function of the role is to provide leadership, management, and communication to and for the team, ensuring the delivery of efficient, effective, co-ordinated and responsive high quality care to patients/clients. and to ensure the delivery of the quality strategy within the designated area incorporating establishment of systems and processes.
