Responsibilities
An Engagement Specialist meets our clients at the crossroads of their mental and physical health to fill gaps and improve engagement and access to care
This position functions as a collaborative member of a client’s care team to facilitate linkage to appropriate services to meet the physical and mental health needs of the client through care coordination, support, education, and motivation
Provides services to ensure/increase access to appropriate care
Serves as a resource/contact to clients in accessing myriad of services within the agency
Facilitates client access to primary/specialty healthcare, communication amongst providers, and referrals to appropriate resources and services within and outside of the agency
Works collaboratively with Wellness Coordinator, Data Management Team, Outpatient Treatment providers, Case Managers, Health Mentors, and other members of the client’s treatment team inside and outside of Porter-Starke Services
Collaborates to implement federal grant requirements, policies, and programmatic changes to develop sustainable improvements to care
Facilitates evidence based groups and creates support opportunities for clients waiting to access care
Manages caseload of clients receiving care management/coordination services
Utilizes assessment and treatment planning tools to maintain a client centered, goal-oriented approach to integrated health
Facilitates identification of high-risk or at-risk consumers that may have unmet physical and/or behavioral health needs
Takes action to engage these consumers to facilitate access to care and resources

Porter-Starke is in need of a FULL TIME Engagement Specialist for our Valparaiso location. Hours are Monday-Friday from 8am-5pm.
Position Summary
Porter-Starke Services is a community mental health center and certified community behavioral health clinic. Our full continuum of care serves adults, families, adolescents, and children with mental health or substance abuse issues. We treat all clients on individual plans of care, tailored to specific needs. An Engagement Specialist meets our clients at the crossroads of their mental and physical health to fill gaps and improve engagement and access to care. This position functions as a collaborative member of a client’s care team to facilitate linkage to appropriate services to meet the physical and mental health needs of the client through care coordination, support, education, and motivation.

Qualifications
• Bachelor Degree in Health Science, Social Work, Rehabilitation, Psychology, Public Health, or related field preferred
• Experience and/or training in Motivational Interviewing and Motivational Strategy Building preferred
• Readily identifies and believes in the holistic benefits of good health
• A commitment to decreased health disparity and social stigma for individuals with mental illness
