Role & Responsibilities:

- Communicating with physicians to discuss treatment plans and progress, and updating medical records accordingly.

- Developing strategies to improve patient outcomes, such as designing exercise programs or teaching effective communication skills.

- Conducting initial assessments of patients' needs and developing care plans that meet those needs.

- Overseeing the delivery of home health services including nursing care, physical therapy, occupational therapy, speech therapy, and nutrition counseling.

- Establishing and maintaining a working relationship with patients and their families while respecting their privacy rights.

- Conducting weekly face-to-face visits with patients to monitor their progress and ensure they are complying with their treatment plan.

- Establishing realistic goals with patients and families regarding home care needs, then regularly evaluating progress towards those goals.

- Communicating regularly with other members of the healthcare team, such as doctors and nurses, to ensure that patients receive consistent care.

Preferred Candidate Profile:

- Bachelor's degree in health care administration, business administration, or related field.

- 1-5 years of experience in a home health Care Management role.

- Strong understanding of state and federal regulations governing home healthcare.
