Position Overview:
As a Home Health Registered Nurse, you'll play a crucial role in administering skilled nursing care to patients in the comfort of their own homes. Working in accordance with physician orders and the plan of care, you'll provide compassionate care under the supervision of our Director of Clinical Services/Nursing Supervisor.
Requirements:
Graduate of an approved school of professional nursing, currently licensed in the state(s) of practice.
Minimum of two (2) years of nursing experience.
Home Health experience preferred.
Ability to exercise initiative and independent judgment.
Primary Duties:
Provide exceptional services to patients based on the individualized plan of care.
Perform initial evaluation visits and conduct regular reevaluations of patients' nursing needs.
Initiate and revise the plan of care as required.
Utilize specialized nursing skills for services requiring substantial expertise.
Implement preventive and rehabilitative nursing procedures as appropriate.
Maintain accurate and timely clinical and progress notes for each patient visit.
Coordinate services effectively and communicate changes in patient condition and needs to the appropriate personnel.
Counsel patients and their families on meeting nursing and related needs.
Participate in and present inservice programs to enhance your knowledge and skills.
Adhere to established Agency policies and procedures.
Collaborate with physicians, process orders, and notify them of patient needs and condition changes.
Determine the level and type of nursing required for each patient and make referrals to specialized therapists as necessary.
Provide supervision and mentorship to other nursing personnel.
Conduct patient care conferences to ensure coordinated care.
Engage in peer review, performance improvement, and utilization review activities.
Demonstrate total patient care when needed, including taking on-call duty as assigned.
Complete OASIS assessments, reassessments, transfers, discharges, and significant change in condition documentation accurately and within designated time frames.
Utilize ICD-+ codes appropriately.
