Job description
The successful applicant will be responsible for working within the Chronic Conditions Program in partnership with the multi-disciplinary team to provide high quality prevention and treatment. They will also be required to develop and maintain a strong referral pathway, work with the broader team including external agencies, and provide a culturally safe, competent and comprehensive care.

Opportunity for on-call and overtime payments.
Ability to salary package up to $15,+00 per annum, plus a further $2,650 per annum on Meals and Entertainment.
Relocation assistance available to eligible candidates.
6 weeks annual leave with 17.5% leave loading.
Retention Allowance $10,000 for each 12-month completion.
