Job Summary:
This permanent part-time position for a Neurological Liaison Nurse offers an exciting opportunity to join our passionate nursing team at MSWA. As a Neurological Liaison Nurse, you will provide expert care to clients living with neurological conditions, making a significant impact in their lives.
Responsibilities:
Manage a diverse caseload of clients, both in their homes and within the broader community, tailoring support to each unique story and journey.
Conduct nursing assessments and develop personalised care plans, ensuring comprehensive care for our clients.
Collaborate with our Allied Health teams to ensure holistic care coordination and facilitate referrals to other health services when necessary.
Travel across the region, from Mandurah in the south to Ellenbrook in the north, to provide essential nursing services and support.
Requirements:
Current registration with the Nurses Board of Western Australia (EN or RN).
Up-to-date First Aid and CPR certification.
NDIS Worker Screening Check (or willingness to obtain).
Full C Class driving licence without restrictions.
Working with children check.
Preferred Qualifications:
Experience in community nursing.
Familiarity with neurological conditions and rehabilitation nursing.
Knowledge of Disability Services Legislation and the National Disability Insurance Scheme.
