We are seeking a skilled Mental Health Nurse to join our team in Flintshire. The successful candidate will be responsible for coordinating care and managing cases for individuals with severe mental health problems.
About the Role
This is an exciting opportunity to work within a multi-disciplinary team, focusing on individuals with complex needs. As a Mental Health Nurse, you will undertake responsibility for the care co-ordination and case management of a defined caseload, adhering to the Mental Health (Wales) Measure 2010.
Main Duties
Provide comprehensive care co-ordination for clients with severe and enduring mental health problems.
Develop and implement effective Care and Treatment Plans, ensuring standards are met.
Act as a point of contact for GP's and other interface services, providing advice, support, and signposting to appropriate agencies.
Formulate and develop standard and complex outcome-focused care and treatment plans.
Conduct comprehensive risk assessments to ensure client safety.
The ability to speak Welsh is desirable for this post, but all applications are welcome from Welsh and/or English speakers.
