JOB PURPOSE

The Human Service Counselor assesses, supports and assists clients / patients within the community or hospital-based setting through the implementation of therapeutic and rehabilitative programs.

QUALIFICATIONS

High school graduation plus an additional human services program/course of over six months and up to one or post-secondary training in a related field or equivalent.
For community based positions in Additions and Mental Health must have 1 year of related experience and posses a valid license.
Ability to attend work on a regular basis.
Ability to perform the duties of the position.
Good work record.
KEY RESPONSIBILITIES

The Human Service Counselor is responsible for:

Participating in the assessment, development and implementation of the client support services plan.
Observing, documenting and reporting, physical, behavioral and psychological changes in the client.
Reviewing, evaluating and modifying individual service plans as part of the multidisciplinary team.
Intervening in crisis situations and providing emotional support to clients.
Assisting clients with activities of daily living by teaching and counseling on lifestyle skills such as parenting, financial planning.
Transporting and accompanying clients to activities and/or appointments related to their care.
Organizing and implementing individual and group therapeutic programs
Assisting clients to access services of various government and community agencies.
Showing others how to perform tasks or duties as part of the departmental orientation.
