Responsibilities
Position Overview: GLSS Home Care Registered Nurse conducts clinical assessments and eligibility determinations to assist the case management team in providing long term services and supports to older adults living in the community
Position Responsibilities: Conducts home visits, nursing facility visits, and medical record reviews and documents determinations according to regulation
The Case Manager Registered Nurse also assesses appropriateness for Personal Care and/or Home Health Aide level of care, and develops personal care plans in accordance with Executive Office of Elder Affairs (EOEA) guidelines

Qualifications: •Currently licensed to practice as a Registered Nurse in Massachusetts, with license in good standing required•Three (3) + years of clinical nursing experience, with at least one (1) year in community or home health preferred•Must demonstrate a passion for supporting consumers in achieving their personal goals related to wellness and community living•The ability to appropriately handle confidential and personal information•Proficient computer skills•Expert time manager•Ability to process information accurately •Reliable transportation and valid driver license to make home visits throughout the GLSS service area•Bilingual candidates will be given priority consideration
