Job Description

Future Wellbeing is a values driven NDIS registered disability service provider based in Sydney South West. Our team is growing and we are seeking an ethical support coordinator to join our close knit team. We aim to make an impact and the successful applicant will be outcomes driven with a dedication to seeing people achieve their goals and dreams.

This is a hybrid/ remote role for a NDIS Support Coordinator, with some face to face meetings as required within the Sydney Metro. The Support Coordinator will be responsible for managing and coordinating support services for clients, assessing client needs, managing budgets, developing care plans, and collaborating with healthcare providers in accordance with NDIS frameworks.

The role also involves providing guidance and support to clients and their families, maintaining accurate records, and ensuring the delivery of high-quality care. You will support a variety of participants from different cultural backgrounds with different disabilities including mental health diagnosis.

Experience as a support coordinator would be highly regarded, however we are happy to mentor and train the right applicant to develop their skills in order to deliver highest quality of service. You will be supported by experienced managers who want to see you succeed in your role.

Skills and Qualifications

Excellent communication and interpersonal skills
Ability to multitask and prioritizs tasks effectively
At least 2 years experience within the disability sector
Knowledge of NDIS including restrictive practices & behaviour supports
Flexible approach to your work & be able to work with a variety of people
Strong organisational skills and attention to detail
Empathy and compassion towards clients and their families
Knowledge of other government agencies and healthcare
Previous experience in a similar role is a plus
Ability to work independently and as part of a team
Lived Experience is highly regarded
Proficiency in Microsoft Office and client management software
Mandatory Requirements

Minimum Cert 4 in a related field
Own reliable vehicle, licence & relevant insurances
· NDIS Workers Screening Check

· WWCC

· NDIS Orientation Modules

Salary range based on qualifications and experience.
