RN- Home Health Case Manager

Overview:
The Registered Nurse (RN) Case Manager coordinates and oversees patient care with the interdisciplinary team. Delivery of care can range from infant stage through the geriatric patient population. Home Health RNs are not just assessing patients but are assessing, the patient support team and their home environment as well, including any social determinants of health obstacles. The RN assists in connecting patients with additional community resources as needed for home management and improving patient compliance and outcomes. The RN will utilize nursing processes in the provision of care and will practice within the scope of the Arkansas Nurse Practice Act. All Home health RNs will follow policies, procedures, and standards of the Conway Regional Health System. The RN will demonstrate a commitment to teamwork through effective collaboration.
SAFETY SENSITIVE POSITION:

This position is a designated as “Safety Sensitive Position” under Act 593 of the State of Arkansas. An employee who is under the influence of ********* constitutes a threat to patients/customers which Conway Regional is responsible for in providing and supporting the delivery health care related services.
Qualifications:
Registered Nurse with current, active license to practice in Arkansas, required.
BSN preferred.
If less than one-year experience as a Registered Nurse, must obtain a BSN degree within 4 years of hire.
Certification Preferred
Current driver’s license in the State of Arkansas
Active personal automobile liability insurance coverage
Minimum of one (1) year experience, preferred.
Proof of high school or equivalent or higher education
Basic computer skills
