About the Role
Monash Health Aged Care Services is seeking a passionate and experienced Case Manager to join our Community Support Options team. Based within our Home Care Package (HCP) program, you will play a key role in supporting older people and their carers across the Casey, Cardinia, Greater Dandenong, Kingston, and Bayside municipalities. This is an exciting opportunity to contribute to high-quality, person-centred care in line with the latest aged care reforms.
Key Responsibilities
Deliver a wellness and reablement-focused model of care, managing a diverse caseload of HCP clients.
Conduct comprehensive client assessments and develop tailored care plans to enable independence at home.
Provide holistic support within a structured case management framework.
Work collaboratively with rostering teams, agencies, contractors, and other stakeholders to ensure seamless service delivery.
Actively contribute to the ongoing development and improvement of the Home Care Program, including service planning and review.
Monitor program outcomes by reporting on KPIs and budget performance to Care Managers and Team Leaders.
Coordinate with multiple service providers to ensure services remain aligned with individual care plans.
Regularly review client needs, well-being, and progress, recommending adjustments to services as required.
Manage new inquiries, referrals, and client information via the My Aged Care Services Portal.
Proactively resolve client-related challenges with practical, client-centred solutions.
Stay informed on aged care reforms, legislation, and quality standards, applying this knowledge in practice.
Prepare, manage, and monitor individual client budgets.
Support clients through the current transition to the Support at Home Program, ensuring continuity of care.
About You:
Relevant tertiary qualifications in health, social sciences, or a related discipline.
Demonstrated experience in case management, ideally within aged care.
Strong knowledge of aged care reforms and consumer-directed care models.
Highly developed computer literacy, communication, and organisational skills.
Ability to work both independently and collaboratively within a team.
A commitment to delivering high-quality, client-centred care with empathy and professionalism.
