Job Description
Provide high-quality customer support to In Home Community Support (IHCS) clients, their families, and the IHCS team.
Coordinate and roster newly onboarded support staff, including arranging buddy shifts to support their transition.
Manage and schedule client services within appropriate IT systems, ensuring accurate documentation of all actions and communications.
Follow up promptly on referrals and service requests to ensure timely and effective care delivery.
Prioritise urgent service and supply requests based on risk assessments and client needs.
Seek management guidance on potential risks or issues identified when engaging with clients or the IHCS team.
Ensure all interactions and record-keeping comply with national privacy and confidentiality requirements.
Liaise with clients and families to explain HealthAbility services and support access to care.
Work within defined role boundaries and escalate matters when outside scope of responsibility.
Participate in a regional rotation alongside colleagues to ensure consistent service coverage.
Desired Skills and Experience
Recent experience with customer service skills, rostering, administration and within in the health/ aged care and or disability work.
A good understanding and knowledge of the broad range of social and health issues to assist clients with service access.
Demonstrated ability to work flexibly, with a strong ability to contribute to and trial new approaches aimed at continual improvement.
Experience working in with the community aged care, community/public health or community-based setting.
A good understanding of health conditions and health system including My Aged Care.
