We’re excited to offer Clinical Care Coordinator and Nurse Mentor opportunities at our first purpose-built, architecturally designed aged care home in Knoxfield, Victoria. We are currently seeking weekend morning (06:30–14:30) and afternoon (14:00–22:00) shift Clinical Care Coordinators and Nurse Mentors. Additional Registered Nurse shifts are also available, providing the opportunity for dual roles. These positions offer opportunities to apply for full-time Clinical Care Coordinator and Nurse Mentor roles, earmarked for early 2026.

The Role

Join the Knoxfield Clinical Care team in a rewarding and fast-paced role as a Care Coordinator and Nurse Mentor. This full time role reports to the General Manager, with support and guidance from the Clinical Care Manager whilst Supervising and supporting our passionate care team. The key purpose of this role is to ensure that all our residents within our beautiful facility in Knoxfield receive the highest quality of clinical care. The Clinical Care Coordinator is responsible for the management and resourcing of the care needs within the facility, documentation and assessments are up to date and in alignment with best-in-class clinical practices.

Key Responsibilities

Provision of clinical expertise and leadership of clinical and care service teams across the Care Home.
Assist Clinical Care Manager to lead carer teams to meet all clinical needs of residents – including monitoring and tracking clinical indicators to achieve best practice clinical care outcomes.
Responsible to manage all aspects of the assessment, documentation and submission of AN-ACC requirements within the facility.
Support the effective documentation and full clinical team user support of policies, procedures, and IT system requirements in the facility.
Coordination of assessment, care planning and review processes within the facility
Support the admission of new residents to facilities for optimal care assessments and initial consultations with resident and families.
Ensure accurate information is recorded and in accordance with all organisational and legislative requirements (AN-ACC, resident records and staff time recording)
Input and oversight into the facility team rosters and ensuring optimal skill mix available for delivery of care to residents.
Manage and document clinical risk within the facility and ensure a commitment to continuous improvements within the facility.
You will offer?

You will have an innate empathy for our residents and be committed to continually improving the quality of clinical care provided to them.
Minimum 1 year experience as a Care Coordinator or similar leadership position
Current unrestricted Registered Nurse Div 1 with Australian Health Practitioners Regulation Agency (AHPRA)
Post graduate qualifications in aged care would be advantageous but not essential.
Experience and extensive knowledge of Aged Care clinical assessments, care plans, AN-ACC and accreditation compliance and documentation.
Engaging style that has ability to easily influence and develop clinical leadership skills across the clinical care team in an aged care environment.
High level of computing skills to ensure that documentation and reporting requirements can be maintained in accordance with organisational and legislative requirements.
A confident team player with excellent communication and interpersonal skills that has a passion for leading clinical outcomes.
Ability to manage multiple stakeholders and have a balanced and level-headed approach to emergency and stressful situations that arise.
Have a valid NDIS Workers Screening Clearance (or ability to apply via How to apply | NDIS).
