Flexible Schedule
Onsite - Travel is required (within a 2-hour radius of Central Indiana) must have reliable transportation
Must have Medicaid Wavier Program knowledge
Ideal candidate will be organized, detail- oriented, have social work, case management or healthcare management experience.

Job Summary:

Works as a Health Coach for 1 Passion to Care Indiana, LLC. As a Health Coach, you will work closely with individuals with disabilities to provide guidance and support in achieving their day-to-day activities. Utilizing your expertise in healthcare, you will develop personalized care plans tailored to each client's needs.

Qualifications/Educational Requirements:

Certification in health coaching or related field preferred
Proven experience in a health coaching role, preferably in home care
Ability to develop personalized care plans based on individual needs
Excellent communication skills and ability to build rapport with clients
Willingness and ability to use mobile applications
Responsibilities/Essential Functions:

The person in this position must be able to perform the following essential job functions with or without reasonable accommodation.

Collaborate with clients, caregivers, and the clinical team to create achievable care plans for sustainable health improvements
Monitor client progress and adjust client specific care plans and caregiver tasks as needed
Create and maintain positive relationships with case managers.
Home visits to clients semi-annually and as needed.
Markets company’s services and increases community awareness
Timely data entry and upload of new packets
Report progress daily to supervisor
Attend daily meetings and present case load information
Timely incident reporting
Maintain a high level of integrity, adhering to ethical standards and HIPPA confidentiality guidelines.
