Overview
We are seeking compassionate and dedicated Home Health Aides to provide exceptional in-home care to clients across various settings. This vital role involves supporting individuals with diverse needs, including seniors, those with disabilities, and patients requiring specialized care such as Alzheimer's or dementia. The ideal candidate will have a strong commitment to patient well-being, excellent communication skills, and experience working in home health or long-term care environments. Duties

Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
Monitor and observe patient conditions, reporting changes or concerns promptly
Administer medications and manage medical documentation accurately in EMR/EHR systems
Perform vital signs checks and assist with catheter care, wound care, or other medical procedures as trained
Support clients with meal preparation and nutritional needs, ensuring dietary restrictions are followed
Provide companionship and emotional support to promote mental well-being
Assist with light housekeeping tasks such as laundry and cleaning to maintain a safe and comfortable environment
Operate assistive devices like Hoyer lifts safely and effectively during transfers
Document patient care activities in compliance with HIPAA regulations and agency policies
Collaborate with healthcare teams including nurses, therapists, and family members to coordinate comprehensive care plans
Qualifications

Prior experience in home health, assisted living, nursing homes, or long-term care preferred
Certifications such as CPR, First Aid are highly desirable.
Experience working with individuals with developmental disabilities, Alzheimer's/dementia care, or hospice is advantageous
Ability to work independently while following detailed care plans and safety procedures
Strong communication skills and compassionate nature essential for patient-centered care
Valid driver’s license preferred for travel between client locations; reliable transportation required
Ability to work flexible hours including live-in employment options if applicable
Knowledge of HIPAA regulations to ensure patient confidentiality at all times
