About the role:

We are seeking a motivated and skilled Grade 3 Allied Health Assistant (AHA) to join our Acute Aged Care at Home program. This role supports older adults to maintain independence and receive safe, high-quality care in their own homes, helping to prevent hospital admissions and functional decline.


Working under the supervision of an Allied Health professional, you will deliver evidence-based, person-centred care, assisting with therapeutic interventions and related activities in line with individual care plans. You will also collaborate closely with the multidisciplinary team, providing both clinical and non-clinical support to ensure seamless, timely care.


Key responsibilities:

Deliver safe, effective, and patient-centred care under supervision, promoting functional independence in older adults.
Collaborate with the multidisciplinary team to support coordinated care in the home environment.
Communicate effectively with patients, families, and team members to enhance patient outcomes.

About you:

You are a motivated Allied Health Assistant with a passion and interest in working with older persons within a home environment.
You have experience as an Allied Health Assistant, preferably in aged care, hospital, or community settings.
You have strong communication, teamwork, and organisational skills.
Commitment to delivering high-quality, evidence-based, person-centred care.
