Coordinate Clinical Care 3 days a week in Wanneroo & Karrinyup
We're expanding!

Access Aged Care is Australia’s leading provider of GP and telehealth supported Aged Care. Our mission is simple: to improve the health and well-being of older Australians by combining compassionate care with innovative healthcare solutions.

We’re expanding and are looking for a dedicated Enrolled Nurse to join our Access Aged Care team in Wanneroo. This nurse will also need to support a busy facility in Karrinyup as well.

What You’ll Do

Work alongside RACF care teams as the on site Access Aged Care nurse.
Facilitate GP rounds (both in-person and telehealth) to ensure smooth, timely care.
Prepare, organise, and triage referrals to maximise efficiency and outcomes.
Conduct bedside assessments and clinical reviews.
Act as the communication bridge between RACFs, GPs, and families.
Deliver excellent customer service, with empathy and professionalism at every interaction
Build strong relationships with residents, families and facility partners.
Use your clinical knowledge and IT skills to ensure quality outcomes.

What You’ll Bring

Minimum of 2 years in an EN/RN role (post graduation) within Australia, and AHPRA registration.
Confident communicator with critical thinking skills
Comfortable working autonomously
Strong organisational skills and attention to detail
Ability to build trust quickly with residents, families and teams
Keen to learn, and confident in your clinical judgement
Technologically proficient - Computer skills are a must!
Reliable vehicle and happy for travel between Wanneroo and Karrinyup
