About the Role
Apoyo Health Services are seeking an experienced Support Coordinator to join our amazing team in Hobart. Apoyo specialises in psychosocial disability and complex case management and prides themselves in having competent Support Coordinators who help participants get the most out of their plans.

In this role you will work with participants to help them understand their plans, how to utilise their funding, connect them with necessary services, provide case management and problem-solving issues as they arise.

You must have relevant experience navigating the NDIS and working between and in multidisciplinary teams.

We offer hybrid working opportunities with time split between working from home, the office and in the community face to face with participants.

Key Responsibilities

Assist participants in understanding and implementing their NDIS plans
Coordinate services and connect participants with appropriate providers
Build strong relationships with participants, families, and service providers
Monitor plan budgets and ensure supports are used effectively
Advocate for participant needs and goals
Maintain accurate records and documentation
Provide case management and direction
Be able to understand and manage needs as they arise and monitor conflicting requests
About You

Genuine passion for working with people with disabilities and improving their quality of life
Strong interpersonal and communication skills
Excellent organizational and time management abilities
Ability to work independently and as part of a team
Problem-solving mindset with a participant-focused approach
Experience in disability services, community services, or a related field (required)
Relevant qualifications (e.g., Community Services, Social Work, or similar) (preferred)
Current driver’s license and access to a vehicle (required)
Valid WWVP card (required)
