Physical Therapist or PTA Home Health

Job Summary:

At Homeside Rehab, we are seeking a dedicated and compassionate Physical Therapist (PT) or Physical Therapist Assistant (PTA) to join our team of healthcare professionals. As a PT/PTA with Homeside Rehab, you will have the opportunity to provide high-quality Physical, Occupational, and Speech/Swallowing Therapy directly in patients' homes, helping them regain independence and return to their community setting.

Responsibilities:

• Conduct evaluations and develop individualized treatment plans tailored to each patient's specific needs
• Provide evidence-based PT interventions to improve mobility, strength, and functional independence
• Deliver pediatric-focused therapy services in the home setting, making a meaningful impact in children's lives
• Maintain accurate and timely documentation, ensuring seamless communication with the clinical team
• Collaborate with the clinical team for coordinated care, prioritizing patient outcomes and satisfaction
• Communicate effectively with office staff, patients, and caregivers, fostering a

positive and supportive environment

Qualifications:

• Active Florida PT or PTA License (Required)
• Preferred: 1+ year experience in Pediatrics, Home Health, Outpatient, or SNF
• Strong clinical judgment and communication skills, with the ability to work independently in a homecare setting

Job Details:

• Job Type: Per Diem
• Starting Pay:
+ PT: $65 per hour
+ PTA: $45 per hour
• Schedule: Set your own hours, with day or evening availability, Monday–Friday with optional weekends
• Location: Coral Gables, FL 33134

What you will do: Responsibilities listed include but not limited to:
• Perform evaluations and develop treatment plans of patients with speech, language, cognitive and swallowing disorders

• Perform discharge evaluations, make appropriate recommendations for home and community safety, and provide referral and education resources
• Communicate patient’s needs and progress to the treatment team, physician, person receiving services and family members
