About the role

This is a full-time Registered Nurse role based in Canberra. As a Registered Nurse at Wellcare Australia Pty.Ltd, you will play a vital part in providing high-quality community-based healthcare services to local residents. What You’ll Be Doing

Clinical Care & Assessment

Assess, monitor, and evaluate the physical and mental health needs of individuals, and groups in the SIL accommodation and community
Conduct mental health screenings, comprehensive assessments, and risk assessments to identify health concerns and support needs
Develop, implement, monitor, and review individualised nursing and mental health care plans aligned with clinical goals
Provide nursing care in community‑based and non‑hospital settings, including acute and ongoing care as required
Monitor patient progress and adjust care plans in response to changes in clinical presentation
Medication & Treatment Management

Administer medications and treatments as prescribed, in accordance with relevant protocols and legislation
Observe, document, and report therapeutic responses, side effects, and adverse reactions
Support medication education and adherence for individuals and carers
Mental Health Support

Provide emotional support, advocacy, and therapeutic engagement with individuals experiencing mental health challenges
Assist individuals to develop coping strategies and self‑management skills
Educate individuals, families, and carers about mental health conditions, treatments, and recovery‑focused approaches
Evaluate the effectiveness of mental health interventions and modify care plans accordingly
Documentation & Professional Practice

Maintain accurate, timely, and contemporaneous clinical documentation and health records
Ensure confidentiality, privacy, and compliance with professional and organisational standards
Participate in shift handovers, team meetings, and clinical reviews
Collaboration & Quality Improvement

Work collaboratively within multidisciplinary teams to deliver coordinated care
Contribute to continuous quality improvement, risk management, infection control, and safety initiatives
Report hazards, incidents, and near misses and contribute to risk mitigation strategies
Education, Leadership & Safety

Provide health education and guidance to individuals, families, and community groups
Mentor students and support less experienced staff as required
Practice safe manual handling, infection control, and appropriate use of PPE
Participate in professional development to maintain competence and AHPRA registration
Values & Compliance

Adhere to organisational policies, values, and mission
Comply with workplace health and safety requirements and legislative obligations
What we're looking for

Current registration as a Registered Nurse with the APHRA.
Minimum 2 years' experience working as a Registered Nurse, preferably in a community healthcare setting.
Strong assessment, clinical decision-making and problem-solving skills.
Excellent communication and interpersonal abilities to work effectively with patients, families and the broader healthcare team.
Commitment to providing compassionate, person-centred care.
Ability to work autonomously and as part of a collaborative team.
Proficient in using digital healthcare technologies and patient management systems.
