Coordinator Transition to Home

Location: Based at Hutt Hospital and working across the CCHV District
Contract: Fixed term (until 30 October 2026), full time

About the Role

Capital Coast and Hutt Valley (CCHV) is rapidly developing and implementing a model for use of 20 Transitional Care beds in Residential Homes across our district. The programme will be titled Transition to Home (T2H) and will run through to November using winter boost funding.

Patients who are medically cleared but with outstanding needs for functional recovery will transfer from Hutt, Wellington and Kenepuru hospitals with Recovery supported for up to 2 weeks by our Advancing Wellness at Home Initiative (AWHI) teams working in partnership with Residential staff, the patient and whānau.

We need a full time Coordinator to assist in rapid development of this stream of AWHI service delivery working closely with AWHI and other staff across the continuum as required.

Main duties will be:

Maintaining a global overview of Residential vacancies, admissions and flow through the Transitional beds
Maintaining a visual whiteboard of patients whereabouts
Liaising with AWHI staff re progress towards EDD/CCDs
Ensuring supports required for a return home are well understood and implemented
Responding to queries from AWHI/Residential care or other staff, escalating as required
Participating in PDSA cycles, reviews and improvements
About the Team/Service Location

AWHI already have well established systems for screening, accepting referrals and offering Rehabilitation to people at home. Now these systems need to be adapted to facilitate transfer to Residential Transitional care beds. Patients will only enter the programme with their consent and may use Residential homes outside of their usual community. AWHI are likely to interface with Community ORA, NASC and HCSS as the client returns home and the Coordinator will ensure this occurs successfully.

About you

The role is suitable for an Allied Health Practitioner or Nurse with experience in Coordination or leadership at a similar level
An understanding of Early Supported Discharge service delivery
Experience in rehabilitation including in Residential homes
The ability to build relationships quickly
An eye for detail
The ability to see solutions and implement process improvements
Flexibility and a 'Can do' attitude
