About the Role
We are seeking a passionate and experienced Welfare Worker to join our team. This role involves delivering person-centred, strengths-based support to people with disability, ensuring services are aligned with ACWA Practice Standards and NDIS requirements.
Key Responsibilities
· Conduct client assessments and develop individualized support plans
· Provide case management and ongoing support to participants
· Advocate for client needs and facilitate access to services
· Promote independence, choice, and control in line with NDIS principles
· Maintain accurate documentation and reports
· Work collaboratively with families, carers, and multidisciplinary teams
· Identify and manage risks, ensuring safeguarding and duty of care
About You
· Qualification in Community Services, Disability, Social Work, or related field
· Experience working within disability services (NDIS experience preferred)
· Strong understanding of person-centred and trauma-informed practice
· Excellent communication and interpersonal skills
· Ability to manage complex client needs
Requirements
· NDIS Worker Screening Check
· Working With Vulnerable people card
· National Police Check
· Current Driver’s License
· Eligibility for or membership with ACWA
