Located in Claremont, St Louis Home Care, is seeking an experienced and compassionate Registered or Enrolled Nurse with aged care or community care experience to work a Monday, Wednesday and Friday morning, with flexibility in hours.

Your responsibilities will include providing clinical care services for Home Care package and private clients, and monitoring, and reviewing clinical governance practices to meet best practice.

The successful candidate will be nurturing, flexible and willing to undertake a wide range of tasks. You will have responsibility for overseeing clinical governance and providing clinical care services.

RN/EN Responsibilities:

· Deliver clinical care needs to clients when required.

. Assess, onboard and review DVA Clinical Care services.

. Regularly review, audit and report on clinical governance practices.

. Provide training and support to the Home Care team on key clinical care competencies and infection control practices.

· Follow up on clients admitted to hospital, and coordinate clients discharge from hospital ensuring all care needs have been reassessed and documented.

· Liaise with My Aged Care and Department of Human Services

RN/EN Requirements:

· Current AHPRA registration

· Tertiary qualification in nursing

· Knowledge of Home Care Packages, DVA Clinical Care services and Aged Care Quality Standards is desired but not essential

· A motor vehicle driver’s license and appropriately insured vehicle.

· National Police Clearance (Less than 1 year old).

· Right to work in Australia.

· Current First Aid Certificate

· Minimum two years experience in aged care/community care.

· Experience with e-Case or Sandwai Home Care Manager System desired.

· Display familiarity and confidence in the use of core office software applications.
