Responsibilities:

• Medication Reconciliation: Conduct medication reconciliation to ensure accuracy and consistency in patients' medication regimens.
• Documentation: Maintain accurate and up-to-date patient records, including medication histories and therapy evaluations.
• Education: Provide education and training to healthcare staff on safe and effective medication use.
• Quality Improvement: Participate in quality improvement initiatives to enhance patient safety and optimize medication use.
• Regulatory Compliance: Ensure compliance with all relevant regulations and standards governing pharmaceutical practice at Max Healthcare.
• Research: Engage in clinical research and stay updated with the latest advancements in pharmacy practice.

Requirements:
• Educational Qualification: B.Pharm or M.Pharm degree from a recognized institution.
• Experience: Minimum 2-5 years of experience in clinical pharmacy.
• Knowledge: Strong knowledge of pharmacology, therapeutics, and clinical guidelines.
• Communication Skills: Excellent communication skills to effectively interact with patients and healthcare teams.
• Attention to Detail: High attention to detail to ensure accuracy in medication therapy management and documentation.
