We are seeking an exceptional Home Health Aide to join our team at HomeCall Frederick, a part of LHC Group.
Job Description

This dynamic position offers the opportunity to build trusted relationships with individuals of all ages while providing in-home personal care and designated health-related services.
• Responsibilities include:
• Providing personal care and assistance with activities of daily living (ADLs) such as mobility, transfer, walking, grooming, bathing, dressing or undressing, eating, or toileting.
• Performing incidental household services essential to the patient's health care at home that prevent or postpone institutionalization.
• Completing clinical notes for each visit within required timeframes, which must be incorporated into the patient's record.
• Care provided according to the aide plan of care/assignment sheet and in accordance with State Practice Act or regulations.
Requirements
• A successful completion of a competency evaluation.
• A current CPR certification is required.
• A current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
