EHR Medical Indexer- post

Medical Records-The primary purpose of this position is to maintain resident medical records and health information systems in accordance with current federal and state guidelines and established privacy policies and procedures.

Respond to requests for medical record information by insurance companies, Medicare, Medicaid, Veterans Administration (VA) or healthcare providers as authorized and in accordance with the Health Insurance Portability and Accountability Act (HIPAA) privacy and security requirements.

Audit and review electronic health record (EHR) documentation for accuracy and completion.

Initiate and set up medical charts for all new patients including all required forms and computer profile setup per facility policies.

Infection Preventionist-The primary purpose of this position is to plan, organize, develop, coordinate and direct the facility infection prevention and control program and its activities in accordance with current federal, state and local standards, guidelines and regulations that govern such programs and as directed by the Administrator and the Infection Prevention and Control Committee.

Plan, develop, implement, evaluate and oversee the infection prevention and control program in accordance with current regulations and guidelines governing skilled nursing facilities.

Ensure that the facility is in compliance with current Centers for Disease Control and Prevention (CDC), Occupational Safety and Health Administration (OSHA) and local regulations concerning infection prevention and control.

Monitor and ensure that all nursing services personnel follow established infection prevention and control practices and aseptic techniques when caring for residents.

Review, summarize and report data relative to key infection prevention and control initiatives including antibiotic stewardship, healthcare acquired infections, immunization programs and outbreaks.

Medical Records-Must possess a working knowledge of medical terminology, HIPAA requirements, coding and indexing.
