As a Registered Nurse (RN) in the Medical Home Port, you will play a vital role in coordinating patient care in collaboration with a diverse team of healthcare professionals
You will ensure adherence to established policies and standards of care while delivering high-quality nursing services to our military families
Collaborate with an array of healthcare professionals to achieve optimal clinical outcomes and patient care quality
Ensure compliance with standards and practices by adhering to established guidelines within the Medical Treatment Facility (MTF)
Conduct physical examinations and gather health histories
Provide health promotion, counseling, and education to patients and families
Administer medications, perform wound care, and implement personalized treatment plans
Supervise and direct the care provided by other healthcare personnel
Assess patient needs and assign care responsibilities based on the skill levels of team members
Respond promptly to emergencies and adverse symptoms
Facilitate communication among healthcare team members to inform care plans and education initiatives
Arrange referral appointments and coordinate specialty care as necessary
Conduct research to improve practices and patient outcomes
