About this Role
We are seeking a Registered Nurse to join our experienced team at our +0-bed Toowoomba Aged Care Service on a night shift basis.

You will work closely with a supportive and friendly team to make a positive impact on the lives of our residents.

Your primary focus will be recognising resident needs and providing assistance that enhances their daily living experience.

Your Key Responsibilities
Conduct baseline and required focus assessments of individual clients' needs, recording findings within a Positive Wellbeing Model
Develop and review care plans based on assessment findings, ensuring care strategies are evidence-based
Evaluate the effectiveness of care strategies in consultation with clients, their representatives, and other members of the care team
Maintain client documentation in accordance with electronic documentation and organisational and legislative requirements
Liaise and collaborate with internal/external service providers in the provision of services

Requirements
A current AHPRA registration as a Registered Nurse
Experience in an aged care setting, showcasing strong clinical assessment skills and the ability to make sound decisions in a dynamic environment
Excellent communication, teamwork, and interpersonal skills, complemented by a caring and compassionate attitude towards our elderly residents
