As a Clinical Case Manager, you will be responsible for overseeing the delivery of care to residents, ensuring their physical, emotional, and social needs are consistently met. You will work closely with residents, their families, and healthcare professionals to develop and implement personalized care plans.

Key Responsibilities

Develop and implement personalized care plans in partnership with residents, their families, and healthcare professionals.
Maintain accurate records of care plans, government-funded home care packages, health status changes, and any interventions provided.
Collaborate with key organisational stakeholders to address and support resident care needs.
Requirements

To be successful in this role, you will need:

Bachelor of Nursing
Current AHPRA registration
Demonstrated experience as a Registered Nurse within aged care
Knowledge of relevant legislation, regulatory requirements, professional standards, policies, and procedures
