**Job Summary**
The Chronic Kidney Disease Care Nurse is responsible for providing high-quality care to clients with chronic kidney disease. This involves developing and implementing individualized care plans to prevent disease progression and promote healthy behaviors.
As a key member of our team, you will work collaboratively with other healthcare professionals to ensure seamless delivery of care. Your expertise in chronic disease management and knowledge of Aboriginal health issues will be invaluable in this role.
**Responsibilities and Requirements**
Develop and implement individualized care plans for clients with chronic kidney disease
Provide education and support to clients on disease prevention and health promotion
