About the Role
Clinical Care Coordination Responsibilities:
Create and implement clinical care plans tailored to individual clients' needs.
Provide high-quality clinical care to our clients, including managing their conditions and developing effective treatment strategies.
Organize and facilitate allied health services for our clients, such as physiotherapy and occupational therapy.
Liaise with care workers, providing training and guidance as needed to ensure they deliver exceptional care.
Oversee and implement quality improvement initiatives to maintain high standards of service delivery.
Promote a customer-focused culture within the team.
Ensure all documentation, procedures, and reporting meet relevant guidelines and legislation.
Maintain accurate and up-to-date client care plans, records, and administrative requirements in accordance with policies, procedures, and funding guidelines.
Comply with relevant legislative requirements.
Requirements:
Current AHPRA Nursing registration.
Driver's license and access to own vehicle.
Professional verbal and written communication skills.
High level of organisational skills.
Sound computer skills in Office and Excel (or similar).
