Job Description
We are seeking a compassionate Registered Nurse (RN) to join our team as a Hospice RN/ RN Case Manager.

The ideal candidate will have:

Experience working in hospice, home health, ICU, geriatric, or oncology
A strong ability to provide comfort, peace, and dignity to patients and their families
Excellent communication and interpersonal skills
A flexible schedule and ability to work independently
Responsibilities include:

Reporting directly to the Administrator, Executive Director or Patient Care Manager
Maintaining primary control and professional management of each patient
Providing direct patient care services and supportive care to the patient's family
Empowering patient's caregivers by educating them on care for their loved one
Managing and collaborating with an interdisciplinary group to provide comfort, peace, and dignity
