Summary
The RN Care Coordinator is responsible for day to day management of patient care from initial contact through recurring visits. Assures patient care services are timely and provided at the appropriate level of care. The primary goal of this role is to support and engage in activities that result in effective patient care and effective use of resources along the care delivery continuum.
Minimum Education
• Graduate of an accredited school of nursing, BSN preferred.
Minimum Work Experience
• 2 years related clinical RN experience per specialty area.
• Nursing charge or lead experience strongly preferred.
• Experience educating a diverse population on healthcare issues highly desirable.
Required Licenses/Certifications
• Current Vermont RN license.
• BLS Certification through American Heart Association
• Valid Driver's License (if conducting home visits).
• Department specific Certifications may apply.

• Demonstrates safe knowledge of nursing practice skills at all times based on theory and consistent with policy and procedure.
• Demonstrates specialty knowledge and practice of nursing.
• Excellent verbal and written communication skills.
• Strong physician collaboration skills.

• Demonstrated organizational and planning skills.
• Ability to effectively communicate and relate information to individuals of varying ages from diverse socio-economic backgrounds.
• Demonstrates moderate knowledge of basic computer skills.
Pay Range: $36.15 - $61.15 #PM24
