Lead the charge in Case Management, creating a person-centered experience that’s efficient, effective, and totally tuned into meeting the unique needs of each client
Collaborate with clients to develop personalised Case Management goals, helping them sustain their tenancy and achieve long-term, sustainable housing!
Craft a clear pathway to permanent housing, while connecting clients with the right health care support tailored to their needs
Build meaningful partnerships with agencies and services, forging a network of support to create the best possible outcomes for clients
Empower clients to engage in community life, opening doors to new social networks and connections that enhance their well-being and long-term success
What do you need? Here’s the checklist for success!
Relevant tertiary qualifications (Social Work, Nursing, or a similar field)
Proven experience in case management – you know how to get things done!
A solid understanding of the issues facing individuals experiencing homelessness, women and children affected by domestic violence, health challenges, mental health challenges, and other complex matters (or an eagerness to learn and grow in these areas)
Experience working effectively with Aboriginal and Torres Strait Islander communities (or a passion to build your skills and cultural awareness)
Collaboration skills – you’re a pro at building partnerships and creating positive outcomes through teamwork
Top-notch communication skills – you can connect with people from all walks of life, adapting your approach to meet diverse cultural needs and creating meaningful relationships
