Job Description

High-level high-quality evidence-based clinical Social Work and leadership services as a member of the multidisciplinary mental health team to consumers aged over 65, with complex and severe mental health disorders. Care is delivered with a focus on planning and provision of clinical and therapeutic interventions within recovery and trauma informed frameworks.
Clinical leadership within the team, and clinical practice and professional supervision to less experienced Social Workers to ensure the maintenance of professional clinical standards within the service.
Support and guidance to HP3 Social Workers within MNMH Older Persons care settings.

About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

Apply high-level knowledge, skills and independent judgement in the assessment and treatment planning for consumers over 65 years with multimorbidity and complex needs and their families/carers, in accordance with the Australian Association of Social Workers Professional Practice Standards and Code of Ethics
Engage in varied and complex inter-sectoral linking and liaison with relevant stakeholders and partnerships, including communication and interpretation of complex clinical information to secure the service needs of consumers.
Provide high-level clinical leadership and participate in the identification, development, implementation and evaluation of quality, service improvement and/or research activities that enhance the delivery of Social Work services, procedures and protocols that support the continuum of care, improve consumer care, and enhance the delivery of work unit and profession specific clinical services.
