We are seeking an experienced tertiary qualified healthcare professional (e.g., nursing / social worker / allied health) dedicated to supporting people impacted by blood cancer through providing information, practical support and emotional support. This role will be responsible for supporting the Sydney metro area and visiting the associated treating centers.



As a Supportive Care Case Manager, you will apply your skills and experience to deliver comprehensive services to people living with or impacted by blood cancer including emotional, practical and information support through telephone, videoconferencing, face-to-face and digital interactions. The primary objective is to enhance the quality of life and well-being of consumers.



This is a permanent full time or part time position at 3-4 days a week with flexibility around which days and hours are worked across the week. . This role will be part of a national Clinical Services team; however, it will be based out of our Westmead Accommodation site in Sydney. This role also has flexibility to work from home up to one day a week if desired.

Providing best practice supportive care and effective coordination and integration of supportive care services to ensure better outcomes for patients.
Increasing awareness and understanding of blood cancer and supportive care options among patients and their families.
Supporting people who are impacted by blood cancer to better able to manage their own physical, mental, emotional, financial, and social well-being.
Providing optimal consumer experience and engagement through personalized care and support.
Ensuring continuous improvement in service delivery through regular evaluation and feedback.
Ensuring compliance with all relevant professional standards and regulatory requirements.


Does this sound like you?

Essential

Tertiary qualification in social work, occupational therapy, physiotherapy, nursing, psychology, or dietetics.
Australian Health Practitioner Regulation Agency (AHPRA) registration or eligibility for membership of respective professional body.
Comprehensive understanding of blood cancer and its impact on consumers.
Knowledge of supportive care services and resources available to blood cancer patients.
Strong interpersonal and communication skills, with the ability to provide compassionate and empathetic support face to face and over telephone or digital modalities.
Excellent organisational and time management skills, with the ability to prioritise and manage multiple tasks.
Proficiency in using IT systems and maintaining detailed and accurate patient records.
Demonstrated ability to work effectively both independently and as part of a multidisciplinary team.
High level of emotional resilience and ability to handle sensitive and distressing situations.
Commitment to continuous professional development and improving consumer outcomes.
A current driver’s licence and access to own vehicle needed (travel reimbursement available)
A valid working with children and vulnerable people clearance or the willingness to obtain
Desirable

Experience in haematology/oncology settings and or supportive care in cancer services.
Knowledge of community resources and support services for individuals with cancer.
Training in counselling or psychosocial support.
