The Role

We are seeking an experienced Clinical Case Manager (Registered Nurse) to join our team in Canberra. As a key member of our care team, you will be responsible for assessing and meeting the physical, emotional, and social needs of residents.

Key Responsibilities

">
Develop and implement personalized care plans in partnership with residents, their families, and healthcare professionals.
Maintain accurate records of care plans, government-funded home care packages, health status changes, and any interventions provided.
Collaborate with key stakeholders to address and support resident care needs.
Assess eligibility and manage residents' access to government-funded care programs.
Requirements

">
To be successful in this role, you will need:

Bachelor of Nursing
Current AHPRA registration
Demonstrated experience as a Registered Nurse within aged care
Knowledge of relevant legislation, regulatory requirements, professional standards, policies, and procedures
Strong written and verbal communication skills
National Police and NDIS clearances
