About the role and where you will be working:
The Support at Home Transition Lead is responsible for leading the transition of the Home Care business to Support at Home, with ongoing responsibility to ensure changed practices are embedded.
The role works closely and under the guidance of the General Manager Home Care, who is accountable for the transition and ongoing effectiveness of the operational performance of the business unit.
This position is located in our central office in Rivervale with the option to work from home 2 days per week. This is a max term contract for 6 months.
Your responsibilities will include;
Lead the development of a modern and contemporary service in the context of the Support at Home.
Create and execute an approach to the provision of Support at Home that positively transforms the experience of our consumers and employees.
Develop key performance indicators and targets that enable the Support at Home leadership team to deliver the commitments made in strategic, operational, team and individual performance plans.
Responsible for the planning, management and delivery of strategic outcomes for SCC (WA)’s Strategic Plan, as they relate to the transition activities, and works collaboratively with employees to achieve those outcomes,
Communicating with the Executive and the Senior Leadership team to ensure they remain informed on significant operational issues and transitional challenges/
Ensure the business unit is compliant with all relevant regulation and take steps to review and monitor compliance.
About you
The ideal candidate will have experience working in Community Care and a solid understanding of the new Support at Home program.
To be considered for this role you’ll need:
Experience in leading a significant operational change to diverse consumer and employee group with 5 years or more experience.
